Green Card Lottery Application Instructions

1. Complete the Application using your key board. Following these guidelines:

Date of birth should be written as follows:

Day, Month (in words), Year -- Example: 15 November 1961

For Place of Birth: City/Town, District/Province, Country

EXAMPLE: Munich, Bavaria, Germany

The name of the country should be that which is currently in use for the place where the applicant was born (Slovenia, rather than Yugoslavia; Kazakstan rather than Soviet Union, for example).

If the applicant is claiming nativity in a country other than his or her place of birth, this country must be listed. If your birth country is the same as native country leave the -- Native Country Field -- blank.

Unmarried children under 21 may be included on each of their parents' application forms. All children over 21 years of age must file separate applications.

The mailing address must be clear and complete, since it will be to that address that the notification letter will be sent if you win. Your welcome to use our mailing address, if you want to protect your privacy OR you may change your mailing address in the future. Our office will contact all winners that use our address. If you use our address, it is your responsibility to notify us of any change in your address or contact information.

2. Print and sign the application

If you wish you can print two copies of the application: the first copy you fill completely the second copy you leave blank with only your signature on it. If needed we will neatly type your information provided in the first copy on the blank copy.

You must personally sign your application. Failure to personally sign your application will disqualify you.

3. Photograph

Recent photographs of the applicant and his/her spouse and each child, including all natural children as well as all legally-adopted and stepchildren, even if the children no longer reside with the applicant, must be attached to the entry. The name and date of birth of each family member must be printed on the back of their photo. Group or family photos will not be accepted; there must be a separate photo for each family member. Photos should be attached with tape and not stapled to the entry. If there is insufficient room on the front of the entry, applicants may tape photos to the back of the page.

SIZE: 2 inches (50mm) square, with the applicant’s, spouse's, or child's name and date of birth printed on the back.

  • Applicant, spouse, or child must be directly facing the camera; the head of the person being photographed should not be tilted up, down or to the side, and should cover about 50% of the area of the photo.
  • The photo should be taken with the person being photographed in front of a neutral, light-colored background. Photos taken with very dark or patterned, busy backgrounds will not be accepted.
  • Photos in which the face of the person being photographed is not in focus will not be accepted.
  • Photos in which the person being photographed is wearing a hat or glasses with dark lens or in which the face is obscured in any way will not be accepted.
  • Photos may be either color or black and white.
